Transcript Request Form

Please fill out the form to the right to request a transcript. Official transcripts are sent directly to the organization. Transcripts sent directly to the student are unofficial. Students must request their own transcripts.

Please note that transcript requests may take 3-5 business days to process.

What is a transcript?

A transcript is a record of your academic accomplishments. Your MacDuffie transcript lists every class you took, when you took them, and the grade you received in each class. Your transcript also displays your cumulative weighted GPA while enrolled at MacDuffie. In addition to your academic information, your transcript includes identifying information such as your name and address at the time you attended the School, date of birth, and date of graduation.

What is an "Unofficial" Copy used for?

An Unofficial copy of your transcript is for your personal use.

  • Your transcript can be used to verify your name, address, and birthdate.

  • Your transcript might be required to apply for scholarships, internships, and jobs.

What is an "Official" Copy used for?

An official copy of your transcript is signed and embossed with the school seal. It will be sent directly to the requestor. This process ensures that the information has not been tampered with and that the transcript contents are still considered official.

  • If you transfer high schools, your transcript will be used to provide the new school with information on what classes you have taken and how many credits you have earned. In this situation, your "Official" transcript will be sent to your new school by the Registrar.

  • You will need an official transcript from our school for all college applications.

  • Some scholarships require an official copy of your transcript for application.

  • Some jobs will require an official copy of your transcript.

Please note that the Registrar cannot provide official records of grades from anywhere but MacDuffie itself. If you need transcripts from other institutions that you attended, you must contact them individually.
